You can apply by any of the following methods:

• Print the Membership Application and the W-9 Form and complete them. Please complete the forms carefully, we are unable to open accounts if the application is not completed in full. Mail the forms along with a photocopy of a current driver's license or Texas ID and a minimum required of $5.00 for savings and $25.00 for a checking account to:

• If account owner is an IBM Employee, a copy of the IBM Badge is also required.
• If you are unable to print or fax the requested documents listed above, call to request a New Member packet to be sent to you.
